Unlimited Pathways to Care provides personal care assistance for all ages and those who are eligible with the State of Texas Medicare and Medicaid program. We mainly target individuals that do not have the support or that require additional support to complete basic daily activities of living. We are here to restore quality of life with the dignity and respect everyone deserves. We work to encourage our clients to learn skills that will increase their level of functioning. We allow clients to move at their own pace and ensure readiness to work towards goals established in their plan of care.
Unlimited Pathways to Care understands that each person’s path is unique. Thus, we have professionals who can determine and identify the necessary steps to take in order to help an individual overcome the challenges and struggles they face.
Our Mission
Our mission is to encourage and empower change, not only in the persons served, but also in the personal care field. By providing quality services, our agency will be a steady resource that will continuously aid in our clientele reaching and maintaining their desired level of functioning. We will ensure clients are always treated with dignity and respect. We also aim to ensure clients understand the importance of their role to impact change in their own lives. We will be a support system and a courier of hope to enhance the quality of life of persons needing personal care and assistance.
